Entering Lat/long on application for airspace COA/COW

Good afternoon all, I am applying for a class D authorization / Waiver and requesting to fly writhing 2 to 3 miles of a D airport. The problem I am having is when I go to put in the Lat/long in the required fields. It continues to give me a error and won't send.

Can anyone give specific guidance on the proper format.

I am getting the lat / long info from fore flight.

I really appreciate any assistance someone may have.

After you enter the degrees, minutes and seconds in their respective boxes, you actually have to type in N and W even though they show in the box.

That's always been a pet peeve of mine. Those greyed out images are just there as a reminder but until you actually type them into the box they have no value assigned to them.
